The Community Care Program provides statewide information, support, and education to caregivers, family, and other loved ones taking care of an individual with memory concerns, dementia, or Alzheimer’s.

Case Management

  • The Idaho Community Care Program provides statewide case management services to caregivers who are caring for an individual with memory concerns, dementia and Alzheimer’s Disease.

     

  • Community Care Managers provide discussion and assessment to determine which resources best suit the caregivers’ needs.

     

  • These options include referrals to local resources, available caregiver services, programs and events.

     

  • The Community Care Manager provides follow-up and monitoring to ensure needs are met and is the go-to person along the caregiving journey.

Advocacy

  • The Idaho Community Care Program works to ensure the development of an economic, social, and health care infrastructure equipped to serve caregivers.

     

  • Community Care Managers provide ongoing advocacy by providing education to the needs of caregivers with employers, health care providers and the community.

Why It Is Important to See Yourself as a Caregiver

Being a caregiver gives you some rights and authority when dealing with health care and social service agency staff.

  • As a family caregiver, you have the right to get information about your family member’s condition.
  • You have the right to be involved in decision making about your family member’s care.
  • You are an essential partner in the health care team and have the right to be trained to provide care.
  • As a caregiver you can find support services that you might otherwise lack.

Respite

  • Respite is a short-term, temporary relief for those who are caring for a loved one.

     

  • Respite can be for a few hours or all day. It can be occasional or ongoing.
     
  • Taking care of a loved one can be a rewarding job but it’s also stressful and it doesn’t offer regular breaks or vacation.

     

  • Respite helps relieve stress, reduces feelings of isolation, and improves can improve family relationships. Everyone needs, and deserves, a break sometimes!

Education

  • The Idaho Community Care Program works to ensure caregivers are aware of opportunities and resources available to continue their caregiver journey.

  • Community Care Managers provide information and referral to available caregiver support groups, Powerful Tools for Caregivers and community resources.

Idaho Community Care Program Mission

The mission of the Community Care Program is to support caregivers of persons with Alzheimer’s Disease and Related Dementia to ensure the highest quality of life possible while living at home.

In order to accomplish this mission, the Community Care Program provides:

  • Information, support and education to caregivers, family, and other loved ones taking care of an individual with memory concerns, dementia, or Alzheimer’s.

  • Provide caregivers with the tools and services they need to effectively navigate their health and life options with care management and consumer-directed options.

  • Extend the reach of community care services into underserved rural communities and those in greatest need.

  • Reduce disparities, caregiver burden and prevent or delay the need for a higher level of care for the care receiver.
